Registered Nurse Evaluation and Monitoring All new clients are welcomed to Atlantic AllCare’s family and services by a Registered Nurse. A complete medical history is gathered to include a list of all medications and treating physicians. Assessment of the home environment is done for possible modifications and/or equipment needs to enhance safety and promote independence. Nutritional assessment and dietary teaching is done to enhance and promote optimal health and well being. Physical assessment is done to include vital signs, heart, lung, abdomen, skin checks, difficulties with balance, mobility, dietary needs as well as needs for assistance with shopping and transportation. With the data obtained, the Registered Nurse develops a plan of care with the client and/or family members to assure all needs medically, personally and socially will be met.
